SCHOOL MEDICINE RECORD

It is now possible to download the 'School Medicine Record' from the website.

This form should be used and filled in whenever your child needs to be given any medication other than that already signed for on the back of the Essential Information Form. This is necessary to enable Matron to legally administer medicine. Please click here to download the form.
